Transmural care
Our department of Transmural Care ensures that hospital care is arranged smoothly to your home. We will do our best to make the continuity of that link in healthcare as strong as possible. In this type of care we work together with hospitals, rehabilitation centers, nursing homes and informal caregivers to jointly organize the care for each client. The goal of transmural care is increasing efficiency, customer satisfaction and quality improvement and to reduce healthcare costs as well.
The transfer nurse discusses the care in the hospital with you and the hospital staff, ensures the timely presence of medical aids and adjustments at home and maintains contact with the health insurer. Depending on your personal needs, customized care is offered. After processing the request for care, the intake process is started and care can be provided, depending on the health insurer.
