Our Transmural care department ensures that care runs smoothly from the hospital to the home situation. We do our best to make the continuity of that link in healthcare as strong as possible. In this form of care, we work together with hospitals, rehabilitation centers, care homes and informal carers, among others, to jointly organize the care for each client. The aim of transmural care is on the one hand to increase efficiency, customer satisfaction and quality improvement and on the other hand to reduce healthcare costs.
The transfer nurse discusses the care in the hospital with you and the hospital staff, ensures the timely availability of aids and adjustments in the home situation and establishes contact with the health insurer. Depending on your personal needs, tailor-made care is offered. After processing the request for care, the intake process is started and, depending on the health insurer, the care can start.
