District nursing is professional care in your familiar environment and this care is self-evident, based on quality of care in which the satisfaction of the client is central.
Our goal is that people can stay home for as long as possible and get their necessary care there. We also want to ensure that people can return home (faster), for example after admission to the hospital. District nursing is for everyone.
We offer the following services:
- Nursing care such as wound care, medication administration (based on protocols) and personal hygiene.
- Encouraging our clients to maintain their independence as much as possible.
- Preventive care and guidance for the client and the informal caregiver by providing information about the course of the disease, nutrition, use of medication and adapting other lifestyle rules.
We have drawn up delivery conditions for the provision of our care. Please take into account our delivery conditions. One example of this is leashing dogs for the safety of our employees.
How can you apply for district nursing?
If you are still in hospital, our transfer nurse will help you prepare for care at home. If you are already at home, you can request home care through your GP. The general practitioner must grant permission via the care portal.
